CHART's Funding Procedure

Please be sure to contact CHART by email (maryannchart@msn.com) or phone (520-749-2151) to be sure funding is available before completing the following process. Currently we are only able to qualify families who live in Arizona or Colorado who have adopted children 9 to 13 years old with major depressive disorder or mood disorder diagnosis.

The Process

Arizona and Colorado families will be asked to complete the initial CHART Application Form on the web site.

  1. The web site will give you information on how to find and use the services of an educational consultant, basic questions to ask the Residential Treatment Centers (RTC), the Compass Evaluation (which CHART will fund) to help determine your child’s needs, and where and how to find an RTC. Once you have narrowed down your RTC selection CHART provides an in-depth RTC questionnaire.
  2. Personnel from CHART will be available to help answer questions to expedite the process.
  3. Once you find an Education Consultant the Educational Consultant may bill CHART for up to $500 of hourly time.
  4. Once an RTC has been chosen, the RTC must fax or email a acceptance letter for your child to CHART along with any insurance information that may be available.
  5. If the family has no access to a computer this information will be forwarded to them by mail.
  6. Once an RTC has been chosen, the cost of the RTC and any insurance information should be sent to CHART by mail. CHART will pay the RTC directly on a monthly basis for a maximum of 3 months.
  7. Funding as available will be awarded based on a first come completed application basis.
